You are not required to submit any medical documents compulsorily when applying for your Dubai transit visa. You may be required to submit a negative Rt-PCR test however.
There are options for a 48-hour transit visa or a 96-hour transit visa available for Costa Rica citizens.
If you get a transit visa for your layover in Abu Dhabi, you will have the freedom to use those 5 hours as per your wish. You can go out and explore the country, eat something delicious or do whatever you wish. Not having a transit visa will limit your presence at the airport before the immigration counter.
